Forgot User Name/Password

If you have already registered, and are unable to log in:
  1. Call your doctors office.
  2. Be ready to provide your personal information for verification purposes.
  3. Tell them you are unable to access the patient portal.
  4. Ask them to confirm the spelling of your first & last name, date of birth, SSN
    and/or your Primary & Secondary Insurance ID.
  5. Once all elements are verified to be correct, ask them to reset your web portal registration.
  6. After your registration is reset, they will give you a new Initial Password.
    Please write it down as you will need it to register.

Once registered, please write down your new email & password.
